IN ORDER TO RECEIVE YOUR YEAR-END THANK YOU! BONUS YOU MUST:

ONE: Fill out the personal information. (Yes, we already have that information, but the IRS requires it and it will just make giving you your THANK YOU! money that much easier for us to do.)

TWO: Indicate how you want to receive your bonus (paper check or direct deposit).

THREE: Choose how much of your THANK YOU! bonus you would like to go into your Hogan 401(k) account.

NOTE: If you are currently participating in Hogan’s 401(k) program, you must fill out the 401(k) form section whether or not you elect to have any of your bonus put into your 401(k) account. Filling this form out at this time will not affect your regular 401(k) contribution amounts.

FOUR: If you elect to be EXEMPT regarding tax withholdings, you must fill out a new W4 form. This form, however, will ONLY apply to this THANK YOU! bonus amount and will not affect your regular paycheck withholding amounts.

FIVE: E-sign, date, and submit.

NOTE: There is a “Save and Continue Later” option at the bottom of the form in case you cannot finish this in one sitting.

And, again, your year-end THANK YOU! bonus will necessarily be held up until we receive this form (by hitting SUBMIT) AND the W4 from Section FOUR if required.
